Position Overview

As a member of the clinical team, completes rehabilitation assessments, participates in the service planning process for all recipients on assigned team, and implements services to recipients in accordance with their individualized service plan (ISP). Communicates and documents recipient’s progress toward their recovery. Coordinates and monitors rehabilitation-related services for recipients with serious mental illness. Works with recipients who have been identified by the clinical team. Works in partnership with assigned RSA Vocational Rehabilitation Counselor, community rehabilitation program providers, and other parties involved assisting recipients to increase or maintain involvement in learning, working, and social activities. The primary focus in assisting recipients is obtaining and maintaining employment and increased community activity involvement.
 
Primary Job Responsibilities
  • Provides continuous evaluation of the effectiveness of treatment through the ongoing assessment of the recipient and input from the recipient and relevant others resulting in modification to the service plan as necessary as related to rehabilitation and employment goals.
  • Pursues best practice outcomes for recipient including continuing education, employment, and socialization.
  • Has knowledge and familiarity of best practices related to employment and utilizes or refers to those best practices as appropriate for individual recipients served.
  • Educates team members as the subject matter expert regarding rehabilitation and vocational related issues and provides ongoing technical training and support to the team as requested or indicated including updates on available community resources, how to utilize the services of the co-located RSA/VR counselor, or co-located vocational providers.
  • Provides guidance, assistance, and support to the clinical team for priority and non-priority population. recipients that are served on designated team as part of the completion of ISP’s and assessments.
  • Documents individual recommendations related to rehabilitation and/or employment and provides signature on assessments and ISPs for all priority population and non-priority population recipients.
  • Conducts an “Orientation to Rehabilitation Services” group bi-monthly to assigned clinical site(s).
  • Develops relationships with community employers, providers, or other organizations to foster an increase in employment and meaningful community activity (MCA) opportunities for recipients.
  • Provides individual and group guidance in identified areas of need as determined by assessments including, but not limited to, utilizing the Readiness Assessment, Functional Assessment, and Skill Development Modules and tools.
  • Assesses, documents, and provides the appropriate level of support and skill needed to assist the recipient to maintain and/or achieve their learning, social, employment, and leisure goals.
  • Provides continuous evaluation of the effectiveness of treatment through the ongoing assessment of the recipient and input from the recipient and relevant others resulting in modification to the service plan as necessary.
  • Pursues best practice outcomes for recipient including continuing education, employment, and socialization.
  • Provides transportation to recipient as appropriate and determined by the clinical team utilizing employee’s personal vehicle. Conducts frequent community visits, including but not limited to, private homes, jail facilities, office/clinic locations, hospitals, and group homes.   • Adheres to minimum productivity and documentation requirements.
Non-Essential Job Responsibilities
  • Ensures confidentiality of verbal and written recipient information, in accordance with HIPPA standards and network policy.
  • Adheres to the legal/ethical/professional guidelines adopted by Southwest Network.
  • Other duties as assigned.
Education, Certification, and Experience Requirements
  • Bachelor’s degree in a social science field or equivalent combination of education, training, and/or experience meeting BMFL standards of BHT equivalent. 
  • Minimum of one year working with SMI adults.
  • Experience with vocational, rehabilitation, educational, and social services for SMI required.
  • Maintain current CPR, First Aid, and CPI Certifications.
  • Must be at least 21 years of age.
 
Knowledge and Ability Requirements

Thorough and working knowledge of psychiatric and co-occurring disorders and best practices relating to rehabilitation is imperative. Knowledge of DSM-IV–TR codes and descriptions, psychotropic medications and crisis intervention. Requires effective interpersonal, written, and verbal communication skills. Ability to follow direction, use of sound judgment, and excellent time management skills are essential. Must maintain good recipient relationships, and be able to identify priorities and stay organized. Preferred typing skills of 25-30 wpm. Knowledge of community based behavioral health services preferred.
 
Tools and Equipment Requirements

Ability to use current electronic records management system. The ability to use a phone, computer, printer, and copier is required. Frequently uses Microsoft Office products, including but not limited to Outlook, Word, Excel, and PowerPoint. The ability to use the internet and various web browser software is required. 
 
Physical Working Conditions and Office Setting Description
Requires prolonged sitting, standing, frequent bending, stooping, or stretching. Some lifting may be required. Frequent and prolonged typing as well as operation of computer, keyboard, and telephones required. Also requires occasional use of fax machines, copiers, security systems, and other office equipment. Work is performed in the community as well as an office/clinic setting. Occasionally exposed to outside weather conditions. Evening or weekend hours are required on an as needed basis.
 
Driving and Other Requirements
Must have and maintain a valid Arizona driver license, have a registered and reliable vehicle that meets Arizona state law standards, and meets the insurance standards of Southwest Network. Position requires travel between Southwest Network sites as well as the community.
