Position Description: Provides Care Management services for older adults and family caregivers, carefully calibrated to the appropriate level of care for the individuals being served. The Care Manager will be responsible for the performance of the General Care Management duties as well as the specific duties as assigned to each level of service.
The primary function of the Care Management Program is to provide participants with professional assessment, service planning, service coordination, referral, and re-evaluation services required for a safe and healthy community life which is manifested through independent living in the community. The development of trusting, supportive relationships with those served is critical, to be achieved through home visits and regular check-ins, resource counseling, and coordinating services. This is done in accordance with the Agency’s vision, mission and values.
Education and Training: Bachelor of Science or Arts degree which includes or is supplemented by at least 12 college-level credit hours in Social Work, Psychology, Sociology, Gerontology, or other behavioral sciences; or Nursing degree with a Registered Nurse License .
Degrees beyond the range listed above require approval of the Chief Executive Officer and may require additional experience from what is described below. All degrees must come from an accredited college or university.
Preferred Experience: One (1) to two (2) years of human service experience; working knowledge of the provision of health care in various settings; knowledge of community resources and care delivery systems; proficiency in use of a laptop computer with MS Office Suite software, relational databases, the Internet, and smart phone technology; case management experience and experience working with older adults.
