Position Overview

Facilitates or assists in facilitation of the strategic planning process of Child and Family Team for assigned children and families. Engages families, stakeholders, informal supports and others involved in the child/family’s life for the primary purpose of developing a strengths-based plan, which meets the family’s needs. The Case Manager I position ensures team participation and ensures relevant expertise is represented on the team to develop and implement an Individual Service Plan (ISP) for each child/family. Responsible for all CFT process related documents including:  Strengths, Needs and Culture Discoveries (SNCD), Individualized Service Plans (ISP), and support plans. In partnership with supervisors, agency staff, and administration, the CM I is responsible for identifying and resolving any barriers that are impeding family’s needs from being met. 



Primary Job Responsibilities

  • Facilitates or assists in facilitation of CFT meetings for assigned families.
  • Facilitates the development of ISP’s using information gained from the comprehensive assessment, the SNCD and input from all team members involved.
  • Provides information, education and support to youth/families, and team members concerning issues of creative treatment planning, coordination of care and the development of informal and community supports.
  • Ensures CFT meets as frequently as necessary in the family’s preferred natural setting to meet their needs.
  • Develops, implements, and revises as necessary, a relevant crisis and safety plan for the family.
  • Works with family to achieve independence/self efficacy,
  • Provides information to CFT stakeholders concerning community resources and linkages to other services.
  • Assists youth and families in securing needed services.
  • Maintains accurate, thorough and current documentation of contact with youth/families, treatment progress and services received/provided and all additional events related to the youth./family.
  • Provides or arranges transportation for youth and families as appropriate by utilizing employee’s personal vehicle or contracted transportation services.  Provides primarily community based services  i.e. visits  including but not limited to, private homes, educational facilities, treatment/service locations, jail/juvenile facilities, office/clinic locations, hospitals, and group homes.
  • Adheres to minimum productivity and documentation requirements.

 Education, Certification, and Experience Requirements

  • Bachelor’s degree in a social science or equivalent combination of education, training and/or experience.
  • Meets OBHL standards of BHT equivalent.
  • Maintains current Arizona state fingerprint clearance card.   
  • Maintains current CPR, First Aid and CPI Certifications.
  • One year of behavioral health experience required.

Knowledge and Ability Requirements

Knowledge of community based and family centered behavioral health services preferred.  Must have knowledge of spectrum of mental illnesses, DSM-IV–TR  codes and descriptions, psychotropic medications and crisis intervention.  Must be able to function as a member of the SWN case management team who facilitates CFT’s for

assigned families.  Must maintain good consumer relationships, possess excellent verbal and written communication skills, and be able to identify priorities and stay organized.

Tools and Equipment Requirements

Ability to use current electronic records management system.  The ability to use a phone, computer, printer, and copier is required.  Frequently uses Microsoft Office products, including but not limited to Outlook, Word, Excel, and PowerPoint.  The ability to use the internet and various web browser software is required. 

Driving and Other Requirements

Must have and maintain a valid Arizona driver license, have a registered and reliable vehicle that meets Arizona state law standards, and meets the insurance standards of Southwest Network.  Position requires travel between Southwest Network sites as well as the community.
