JOB DESCRIPTION:    Community Liaison
PROGRAM:            Project Permanence

Team with family specialist and family in developing a wraparound plan to support SED children to live in the least restrictive environment possible and to address child’s social, recreational, spiritual, and life-skill development.  Ages of children range from 6-17 years.

  • Bachelor’s Degree from an accredited college in psychology, sociology, social work, or counseling with four years documented mental health experience with children, including experience in clinical interventions, setting limits, and crisis intervention.
  • Six months experience providing wrap-around or other community-based collaborative services to children and their families, including participation in Child and Family Teams.
  • Demonstrated awareness of, sensitivity to and competence in dealing with cultural and socioeconomic diversity of client population.
  • Valid California driver’s license, a clear driving record, availability of personal vehicle and personal vehicle insurance coverage.
  • The willingness and the ability to be trained, using our therapeutic and relationship-building philosophy that includes Collaborative Problem-Solving, Life Space Crisis Intervention (LSCI), and SAFE Environments training programs.
  • Physical fitness with the ability to bend, stoop, kneel, drop to one knee and get back up.
  • Must meet eligibility requirements to bill for Medi-Cal services.
  • Create linkage and coordinate resources relevant to the needs of the children and families being served by that team.
  • Participate in the on-call rotation system for crisis response.
  • Evaluate child’s social, recreational, and life-skills functioning.
  • Evaluate caregiver needs.
  • Implement plan goals developed by Family Treatment Team.
  • Help child develop and utilize their strengths, interest, and skills.
  • Design interventions relevant to child’s social, recreational, and life-skills development.
  • Develop a plan with family specialist and caregivers that over time achieves caregiver independence in meeting the needs of their child.
  • Participate in on-going evaluation of program interventions to ensure quality, consistency, and relevancy of interventions to the particular child and family.
  • Identify and link with resources in the community that facilitates goals.
  • Develop collaborative relationships with community resources with MOUs (Memorandums of understanding).
  • Interact effectively with care-giver(s), siblings, and other significant people in the child’s familial and social network.
  • Provide role modeling for caregivers.
  • Assist therapist with case-management tasks.
  • Participate in all educational and child team meetings, with appropriate written preparation when indicated.
  • Assume responsibility for Medi-CAL documentation, as required.
  • Provide crisis intervention, as indicated.
  • Participate in weekly supervision, in external and in-service trainings relevant to the program services, in training and supervisions regarding facilitation of Child and Family Teams.
  • Participate in agency in-service training program. Understand and follow all LCC safety policies, procedures and rules.  Perform all job duties safely.  Attend all required safety training sessions.  Identify and immediately report all workplace hazards, safety rule violations, near misses, and work-related injuries.
  • Other Duties as assigned by the Family Specialist.
CLASSIFICATION:    Regular, full-time, exempt, benefited position.

SCHEDULE:    Flexible schedule based on program and client needs. Generally weekdays from 10:00 AM – 6:00 PM. Weekends as needed.

AFFILIATION:     Non-Bargaining.

COMPENSATION:    Commensurate with experience and salary scale.
