PEER MENTOR 

Position Overview
Encourage, assist, and support the recipient in expressing the recipient’s perspective throughout the processes of assessment, service planning, and service delivery. Be able to create and facilitate a variety of peer group activities that support and strengthen recovery. Develop a knowledge base and contacts in community to assist with providing needed resources to recipients and provide consultation to members of the adult clinical teams on community-based resources. Will attend the recipients’ clinical team meetings in preparation of the transition from Child and Adolescent Services to Adult Services and assist the recipient in identifying recipients’ culture and concerns as they transition into the Adult System of Care.
 
Primary Job Responsibilities
  • Help recipient to combat negative self-talk, overcome fears, solve problems, and articulate, set, and accomplish goals.
  • Teach recipients to advocate for the services that they want/need.
  • Assist recipients in building social skills in the community that will enhance increased employment and Meaningful Community Activity (MCA).
  • Assist clinical staff in identifying program environments that are conducive to recovery; lend their unique insight into mental illness and what makes recovery possible.
  • Develop knowledge base and contacts in community to assist with providing needed resources to recipient; also provide consultation for clinical staff on community-based resources.
  • With assistance from clinical staff, assist the recipient in obtaining information regarding support or advocacy organizations for additional support as appropriate and needed.
  • Communicate opportunities for recipients to become involved in the Mental Health System on a variety of levels.
  • Establish, support, and oversee the Clinic Advisory Council.
  • Welcome new recipients and recipients’ families. Assist in building bridges with clinic staff and overall system partners.
  • Connect recipients with community based resources to facilitate community integration.
  • Explain a Individual Service Plan (ISP) and explain the advantages to recipients, including advance directives and court-ordered treatment.
  • As relevant, participates in the Individual Service Planning process and attend clinical staffing, team meetings, and any other site location meeting.
  • Must document, via electronic records management system, all clinical activities provided to recipients.
  • Conducts Peer Group meetings as needed.
  • Provides transportation to recipient as appropriate and determined by the clinical team utilizing employee’s personal vehicle. Conducts frequent community visits, including but not limited to, private homes, jail facilities, office/clinic locations, hospitals, and group homes.   • Adheres to minimum productivity and documentation requirements. 
     
     
     
Non-Essential Job Responsibilities
  • Ensures confidentiality of verbal and written recipient information, in accordance with HIPPA standards and network policy.
  • Adheres to the legal/ethical/professional guidelines adopted by Southwest Network.
  • Other duties as assigned. 
     
     
     
     
Education, Certification, and Experience Requirements
  • High School Diploma / GED or equivalent combination of education, training, and/or experience meeting OBHL standards of BHPP equivalent. 
  • Must be a current or prior recipient of Maricopa County behavioral health services.
  • Minimum of one year in recovery is required.
  • Peer Support Certificate preferred.
  • Must be at lest 21 years of age. 
Knowledge and Ability Requirements
Requires effective interpersonal, written, and verbal communication skills. Ability to follow direction, use of sound judgment, and excellent time management skills are essential. Must be able to share common experiences with recipients, regardless of circumstances or severity of recipients’ disabilities. Must maintain good recipient relationships, and be able to identify priorities and stay organized. Knowledge of community based behavioral health services preferred.
 
Tools and Equipment Requirements
Ability to use current electronic records management system. The ability to use a phone, computer, printer, and copier is required. Frequently uses Microsoft Office products, including but not limited to Outlook, Word, Excel, and PowerPoint. The ability to use the internet and various web browser software is required. 
 
Physical Working Conditions and Office Setting Description
Requires prolonged sitting, standing, frequent bending, stooping, or stretching. Some lifting may be required. Frequent and prolonged typing as well as operation of computer, keyboard, and telephones required. Also requires occasional use of fax machines, copiers, security systems, and other office equipment. Work is performed in the community as well as an office/clinic setting. Occasionally exposed to outside weather conditions. Evening or weekend hours are required on an as needed basis.
 
Driving and Other Requirements
Must have and maintain a valid Arizona driver license, have a registered and reliable vehicle that meets Arizona state law standards, and meets the insurance standards of Southwest Network. Position requires travel between Southwest Network sites as well as the community.
